Texas Consumer Notice
Rights and Records

In accordance with House Bill 4224, 89th Regular Session, Section 181.105, Texas Health & Safety Code:​

​​1. Clients are entitled to request their healthcare records at any time by submitting a written request via email to Tandem Behavioral Health, PLLC at info@tandembh.com (Note: standard fees and timelines apply).

 

2. If clients have complaints regarding a practitioner, they can submit those to The Behavioral Health Executive Council (BHEC) by submitting a complaint form directly to BHEC.

3. If clients have complaints about Tandem, they can submit those to the Office of Attorney General's Consumer Protection webpage.
